New Planning Body Naming Contest
June 1-30, 2015
What it is: The MN HIV Services Planning Council (MHSPC) and Community Cooperative Council on HIV/AIDS Prevention (CCCHAP) are merging into one planning body focused on care and prevention and the Integrated Planning workgroup is holding a contest to come up with the name of the new planning body.
The contest winner will receive a $50 gift card to Target.
How to enter:
1. Submit your entry or entries by June 30 at 11:59 pm to [email protected] or [email protected].
2. No more than 3 entries may be submitted by one entrant.
3. Include your submission, name, postal address and email address.
Eligibility:
1. The contest is open only to individuals. The contest is not open to companies, educational institutions, organizations, etc. or to groups associated with such institutions.
2. Members and affiliates of the MHSPC and CCCHAP are welcome and encouraged to participate in this contest. However, any member/affiliate who submits a name for consideration will not be eligible to participate in the final decision making process.
3. All entrants must be at least 18 years of age.
Judging and selection of winner:
1. The Integrated Planning Workgroup will decide on the top three submissions to be voted on by MHSPC and CCCHAP members.
2. The top three submissions will be forwarded to MHSPC and CCCHAP for voting by majority rule. The vote for MHSPC is Tuesday, July 14, 2015 during the Planning Council meeting. The vote for CCCHAP will be done electronically between July 13-17, 2015.
3. The Integrated Planning Workgroup reserves the right to not select a winner if, in its sole discretion, no suitable entries are submitted.
4. The Workgroup reserves the right to disqualify any entrant or entry at its sole discretion.
5. The winning submission becomes the property of the new planning body and may only be used for the winner’s portfolio of work.
6. The winner of the naming contest will be required to sign a release giving sole ownership to the new planning body.
7. Accepting the prize constitutes permission for the workgroup to make public and otherwise use the winner’s name for publicity purposes. Further personal data may be requested but is not required.
Acceptance:
Participation constitutes the entrant’s full and unconditional agreement to and acceptance of these official rules. By participating in the contest, the entrant is representing and warranting that he/she has read and understood, and agrees to be bound by, these rules. Including the guides and rules referred to herein, these official rules constitute the entire agreement between the entrant and MHSPC and CCCHAP in relation to the contest. They govern the entrant’s participation and supersede any prior or other agreements between the entrant and MHSPC and CCCHAP and relating to the contest.
